Physical Therapist

We are seeking a dedicated and compassionate Physical Therapist to join our team. The ideal candidate will provide high-quality patient care in both outpatient and acute care settings, focusing on developing and implementing effective treatment plans tailored to each individual's needs. This role requires a strong understanding of physical therapy practices, particularly in geriatrics and pediatrics, as well as experience in discharge planning and care plan development. *Responsibilities*
* Conduct thorough evaluations of patients to assess their physical condition and needs.
* Develop individualized care plans that align with patients' goals and rehabilitation requirements.
* Implement treatment protocols using various physical therapy techniques and modalities.
* Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
* Collaborate with other healthcare professionals to coordinate comprehensive patient care.
* Educate patients and their families on exercises, techniques, and strategies for recovery.
* Maintain accurate documentation of patient evaluations, treatments, and progress notes.
* Participate in discharge planning to ensure a smooth transition for patients from therapy to home or other care settings. *Qualifications*
* Doctor of Physical Therapy (DPT) degree from an accredited program.
* Valid state license to practice physical therapy.
* Experience in outpatient settings preferred; familiarity with geriatrics and pediatrics is a plus.
* Strong knowledge of patient care practices, including acute care environments.
* Excellent communication skills for effective interaction with patients, families, and healthcare teams.
* Ability to develop comprehensive care plans that address the unique needs of each patient.
