This is a full time position being on call 7 days, then off 7 days. You will have a second 7 on/7 off RN working as your teammate with additional support from the Night Team for Continuous Care, Medical Director, Social Services, and the Administrator On-Call.

• Answer calls in a timely fashion with accurate triaging and professionalism to patient/family/facility.
• Make initial visits to patient/family to explain hospice services, evaluates patient for appropriateness, admits patient, and initiates plan of care with authorization of the attending physician and medical director.
• Graduate from an accredited nursing program required.
• At least two years of experience in hospice or home care setting preferred.
