Our program is all-inclusive concierge care led by skilled primary care providers and an interdisciplinary team with 11 specialties represented. Care Resources is seeking a full-time Physical Therapist to join our fabulous team of healthcare professionals This position is responsible for providing physical therapy services to the participants of Care Resources.
This is a Monday - Friday day shift, hourly position. No weekends and no holidays It is based at our multi-disciplined clinic and day center in Grand Rapids and will occasionally include home visits for our participants.
Provides treatment and implements physical therapy procedures
Trains staff and caregivers on follow-through programs
Participates in care plan development and discharge planning for participants
Assesses the need for adaptive durable medical equipment, specialized shoes, and prosthesis
Communicate therapy progress and discharge plans to the team
Assists in training and provides professional supervision for PTA(s) and students
Provides education and training to staff, participants, and caregivers
This position also involves some travel to client homes in Kent County and select zip codes in Allegan, Barry, Ionia and Ottawa Counties.
Schedule/Shift: Monday - Friday Day shift Hourly
Bachelor's Degree from an accredited physical therapy program
