WELLBOUND Certified Home Health Agency is currently seeking a FULL TIME RN-Charge Nurse to join our team to assist with our geriatric patients who reside at an assisted living facility in Huntington. There is 1:1 paid training with the supervisor, supportive staff, and an abundance of available resources.

*Responsibilities for Registered Nurse (RN):*
* Supervision of LPN staff
* Coordination of Care of the patients at assigned facility
* Updating and Implementing MD orders
* Field based Nursing Work (Oasis)
* Completion of SOC/Recertification/ ROC documentation
* FLEXIBLE SCHEDULE
* Coordinate resources & services for assigned caseload; working with the patient and physician
* Must be pro-active and take initiative when dealing with the patients
* Must provide quality and timely documentation after providing care
* Maintain up to date/accurate documentation for assigned cases
* Maintains a cooperative relationship among health care teams

*Qualifications / Requirements for Registered Nurse (RN) :*
