Boulder Community health is seeking a Physical Therapist who assumes responsibility and accountability for providing quality patient care through the provisions of physical therapy services.

Current Colorado state license for Physical Therapy.
•Developing, executing, assessing, and modifying physical therapy intervention as it applies to the neurologically involved patient population.
Selects and establishes physical therapy diagnosis.
•Planning skills: Formulates and maintains an individualized patient plan of care based on long and short-term goals.
•Implementation skills: Serves patient population from adolescent to older adult/geriatric. Is able to modify assessment, treatment and interaction skills based on the age and developmental level of the patient.
•Promotes effective communication and teamwork with team members, the hospital and other contacts.
•Reports and documents evaluations, plans, treatment techniques, assessment, and patient progress.
