Life as a Hospice RN Case Manager Role of the RN Case Manager As a Hospice Case Manager, you will have a special opportunity to provide, coordinate and direct the provision of hospice nursing and palliative care to terminally ill patients in their home and wherever they call home. Our Hospice nurses will closely collaborate with the attending physician, patients and their families, and other members of the patient care team to provide and maintain continuity of patient care to achieve excellent pain and symptom management and high-quality end-of-life care for the patient. We service an adult population providing support to patients and families. Nuts & Bolts of the Job Hospice RN Case Managers work Monday - Friday, 8AM - 5PM with a potential weekday on-call and Holiday coverage schedule. Provide Case Management: Assign Aide Visits Coordination of Care with Social Workers, Chaplains, Volunteers and Bereavement team Develop and nurture relationships with facility staff and leaders throughout the community Enriching and Fulfilling Lives Our hospice care focuses on the whole person - combining medical care and support for emotional and spiritual needs, as well as support for loved ones and caregivers. Responsibilities Performs patient assessments and collaborates with the care team to develop and implement a plan of care. Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so. Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies. Teaches patients, families, and caregivers appropriate patient care techniques. Regularly communicates with clinical manager and care team. Qualifications 1+ year(s) of nursing experience in acute care setting(s), ER or fast-paced environment(s).
