POSITION

I/DD Nurse Coordinator

POSITION PURPOSE

The Registered Nurse will support individuals living with an intellectual and/or developmental disability who reside in a community setting by providing clinical case management and client care oversight. Clinical case management will support service provision focused on the successful achievement of client goals established by the person receiving services and/or his or her family while supporting medical needs and activities of daily living. Examples of support services can include but are not limited to residential care, personal care, assisting with homemaker tasks, providing respite care, mentorship and providing community connections.

KEY DUTIES & RESPONSIBILITIES

  • Perform skilled nursing services, clinical oversight, and case management.
  • Provide ongoing case management and assessment through phone calls and home visits.
  • Carry a caseload of clients receiving IRSS Services and serve as a clinical resource for other I/DD Services.
  • Establish and maintain relationships with consumers and Direct Service Providers.
  • Provide ongoing education on medical care and health and safety techniques to individuals and their families.
  • Educate Direct Service Providers on clinical care, medication, disease processes, and delegated nursing tasks.
  • Assess Direct Service Provider proficiency and ensure they can safely and legally perform assigned duties.
  • Perform nursing delegation in accordance with Board of Nursing Regulations.
  • Write individualized clinical care plans aligned with Physician's Orders.
  • Maintain open communication and collaboration with individuals and their families to manage medical documentation.
  • Update Medication Administration Records as needed.
  • Perform quarterly and annual delegation monitoring, care supervision, and medication administration record reconciliation.
  • Understand and amend Care Plans to reflect updated Physician's Orders.
  • During client onboarding, create Medication Administration Records, identify skilled nursing tasks, and collaborate with I/DD Service Coordinators.
  • Assess and manage medical supplies and assistive technology for individuals receiving services.
  • Be available during business hours to answer clinical questions and provide recommendations.
  • Utilize Service Plans and medical documentation to support writing Individualized Service & Support Plans (ISSPs) and Individual Goal Statements (IGSs).
  • Create, update, and manage various medical documentation and protocols.
  • Ensure high quality and continuity of care by coordinating with I/DD Service Coordinators, physicians, and other care team members.
  • Report key information and changes in client condition to appropriate personnel and Primary Care Providers.
  • Communicate and collaborate with internal and external care teams to ensure effective care.
  • Maintain timely records and comply with HIPAA regulations.
  • Attend nurse and company wide meetings, assimilate new information, and incorporate it into current practice.
  • Complete and submit all required documentation per governing body rules and regulations.
  • Perform other duties as assigned to support quality service to individuals receiving services.

  • Ability to anticipate and respond to the needs of individuals receiving services.

  • Knowledge and skills relevant to providing care and services.
  • Quality of care metrics (infections, wounds, etc.).
  • Customer service and satisfaction feedback from individuals receiving services and their families.

Experience:
  • 1 year in an acute care setting (required)
  • Case management or home health experience (preferred)
  • Experience in intellectual and/or developmental disability services (preferred)
  • Supervisory experience in healthcare (preferred)

Education and Licenses:
  • Current Colorado Registered Nurse license
  • Bachelor of Science in Nursing
  • Current CPR certification
  • Completion of all PASCO-required training

Skills:
  • Bilingual in Spanish (preferred)
  • Strong clinical judgment and communication skills
  • Proficiency in Microsoft Outlook and Electronic Health Record systems
  • Knowledge of medication interactions and therapeutic actions
  • Ability to work with diverse populations and maintain commitments

Physical or Other Requirements
  • Medium detailed work:
    • Requires walking or standing a significant amount of the time.
    • Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
    • Requires stooping, kneeling, crouching, grasping, lifting, reaching, pushing
    • Requires close visual acuity to perform an activity such as: preparing and analyzing data and figures; viewing a computer terminal; extensive reading; visual inspection involving small defects, small parts, and/or operation of machines (including inspection).
  • Access to a reliable transportation, either public transportation or an automobile with a valid Colorado driver's license and insurance
  • Must complete all comp/trainings required by PASCO administration

PAY $36 - $41 + mileage reimbursement and $5,000 sign-on bonus

LOCATION/SCHEDULE Hybrid work schedule Monday-Friday from 8:30a - 4:30p. Tu/Th working from office and M/W/F visiting clients homes. Territory of Coverage will be north of I-70, south of Wyoming and East of the Front Range. This can include travels to Broomfield, Longmont, Boulder, Greely, Westminster, Thornton, Northglenn, Brighton and Commerce City.
