DEFINITION:
Responsible as a member of the interdisciplinary team for the provision of social work services to patients
and their families. The Medical Social Worker must provide initial psychosocial evaluations, ongoing
psychosocial assistance, direct casework services, bereavement services, community education, and
outreach. The Medical Social Worker must offer and provide emotional supportive care in order to
maximize the comfort and health of patients and families consistent with Family Hospice of Northeast
Indiana (FHNEI) Mission, Strategic Plan, and Policies and Procedures.
QUALIFICATION REQUIREMENTS:

* Certificates, Licenses, Registrations:
* You must have and maintain a valid Driver's License, maintain automobile coverage and have access to an automobile during scheduled work hours.
* Education and/or Experience:
* A Master's degree is preferred. A Bachelor's degree or higher in Social Work from a Council on Social Work accredited institution. A demonstrated ability in casework, counseling and group work, including two (2) years of clinical work experience, preferably in either a healthcare or hospice setting.
JOB SPECIFIC DUTIES and RESPONSIBILITIES
* Maintains Social Work Licensure through the Indiana Professional Licensing Agency or actively works towards obtaining licensure within 3 years of hire date.
* Maintains the standards of Social Work excellence in accordance with the principles set forth by the Indiana Licensing Agency, NASW, IHPCO, NHPCO,
* Performs comprehensive initial psychosocial and bereavement evaluations and assists in the development and implementation of goal-directed family care plan.
* Reports pertinent information to the Supportive /or Team Coordinator and members of the interdisciplinary team.
* Conducts ongoing reassessments of patient/family needs and provides assistance as needed.
* Provides short-term crisis intervention and individual or family assistance when indicated.
* Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and facilitate related referrals.
* Observes, evaluates and brings to team conferences information regarding psychosocial, spiritual, physical and financial conditions affecting the patient and family.
* Assumes the active role of advocate for the patient/family unit.
* Documents comprehensive psychosocial information clearly and concisely in a timely manner.
* Records all patient/family visits, telephone contacts and referral actions in the MISYS Record within a period of 48 hours.
* Participates as a member of the Bereavement Team as assigned.
* Performs Enrollment Visits as requested and completes paperwork within 24 hours or the next business day.
* Attends departmental meetings, IDT and other meetings as assigned and appropriate.
* Participates in agency and community programs as requested to promote professional growth and understanding of hospice care.
* Demonstrates familiarity with policies of the agency and rules and regulations of State and Federal bodies, which aid in determining policies.
* Makes all requests (i.e. PTO, schedule alterations) through Supportive Care Director.
* Adheres to and promotes the elements of the Corporate Compliance and HIPAA Programs.
* Assists and participates in orientation and educational programs.
* Participates in Quality Assurance Performance Improvement (QAPI) activities.
* Adheres to FHNEI policies and procedures.
* Attends at least 12 hours of in-service programs during the calendar year.
* Assumes responsibility for own personal and professional development and maintenance in the state of the art skills regarding hospice and palliative medicine.
* Performs other responsibilities and functions as directed.
