Location: Manhattan, Bronx, Brooklyn, Queens, Westchester, Rockland County
Salary: $55.00 - $70.00 per hour + Full Benefits + Sign-On Bonus

Join our compassionate and dedicated team as a Home Health Field Nurse specializing in hospice care. Responsibilities:
  • Provide comprehensive nursing care to hospice patients in their homes across the New York boroughs.
  • Conduct thorough assessments of patients' physical, emotional, and psychosocial needs.
  • Develop and implement individualized care plans in collaboration with an interdisciplinary team.
  • Administer medications, treatments, and other interventions as prescribed.
  • Educate patients, families, and caregivers on disease processes, symptom management, and end-of-life care.
  • Ensure accurate and timely documentation of patient assessments, care plans, interventions, and outcomes.
  • Coordinate care with physicians, social workers, chaplains, and other healthcare professionals to provide holistic care.
Requirements:
  • Valid Registered Nurse (RN) license in the state of New York.
  • Minimum of 1 year of clinical experience in hospice, palliative care, or home health nursing.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and manage time effectively.
  • Compassionate and empathetic approach to patient care.
  • Willingness to travel throughout the New York boroughs to visit patients in their homes.
