DESCRIPTION:
  • Provide regular administrative and clinical supervision to Case Managers and RNs. As part of an interdisciplinary team, staff conduct comprehensive assessments, create care plans and coordinate in-home services via a network of vendor agencies.
  • Responsibilities include training staff, providing timely feedback on performance, and ensuring services are delivered in compliance with program regulations.

QUALIFICATIONS:
  • Bachelor's Degree required.
  • Minimum 2 years of experience in social services field.
  • Supervisory experience preferred.
  • Master's Degree in Social Work or related field preferred.
  • A reliable vehicle, current driver's license and safe driving record required.
