Job Description Summary The Clinical Pharmacist is a licensed professional responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with the policies and procedures of Option Care, pharmacy practice professional standards, and applicable regulatory entities. This entry‑level position is an introduction to the practice of clinical pharmacy in an alternate setting. The Clinical Pharmacist coordinates patient services with the Nursing Department, monitors patient progress toward goals, and educates patients, caregivers, staff and other professionals regarding appropriate medication use. Job Responsibilities (listed in order of importance and/or time spent) Determine the suitability of individual patients for home care; collect and organize all patient‑specific information needed for assessment. Design, recommend, monitor and evaluate patient‑specific pharmacy care plans for the pharmacotherapy of home care patients; make suitable recommendations for modifications in the prescribed medication therapy or monitoring parameters. Modify pharmacy care plans based on evaluation of monitoring data; apply pharmacokinetic principles and assess laboratory values. Provide medication‑use education to Option Care staff, other professionals, home care patients and/or their caregivers using effective patient education techniques. Ensure continuity of pharmaceutical care to and from the home; prepare and dispense medications with appropriate techniques and follow Option Care policies and procedures. Manage the use, maintenance, and troubleshooting of medication administration equipment and related equipment used in the management of home care patients. Respond to drug‑information requests from health care providers and home care patients, formulating responses based on literature analysis. Manage the use of investigational drug products per regulations and Option Care policies; utilize pharmacy support personnel effectively. Comply with accreditation, legal, regulatory, and safety requirements. Maintain confidentiality of patient and proprietary information while observing legal and ethical guidelines. Supervisory Responsibilities No supervisory responsibilities. Basic Education and/or Experience Requirements Graduate of an accredited School of Pharmacy. Current, active Pharmacy license as granted by the State Board of Pharmacy. At least one of the following: One year of experience in sterile IV admixture procedures. Knowledge of non‑sterile compounding, immunizations, and medication therapy management (MTM). Completion of an ASHP‑accredited Pharmacy Residency Program. Completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy. Equivalent experience approved by the VP of Pharmacy and/or SVP of Clinical Services. Basic Qualifications Experience providing customer service to internal and external customers, meeting quality standards for services and evaluating customer satisfaction. Basic skills in Microsoft Word, Excel, and PowerPoint. Travel Requirements Willingness to travel up to 5 % of the time for business purposes (within state). Preferred Qualifications & Interests (PQIs) Completion of an ASHP‑accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience. Experience in home care, hospital or other clean‑room setting. Management Requirements At least one year of experience in indirect management of team members, including assisting in the development, training and assignment of work/projects to other team members. Compensation Salary to be determined by the applicant’s education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data. Pay Range: $112,439.64–$187,406.90.
