Hourly Range: $22.75 USD to $31.63 USD Clinic LPN Job Summary The LPN provides safe nursing care in a timely manner to patients, within the scope of a LPN as defined by the Vermont State Board of Nursing. Pre-requisites Education: Graduate of an accredited school of practical nursing with current licensure as a Licensed Practical Nurse in the State of Vermont. Experience: Five years of experience as a LPN and at least one year in current NMC LPN position. Serves as an active preceptor/mentor and/or super user for employees. Participates in two or more process improvement opportunities within the last 12 months. Proficient to cover other roles throughout the department and cross trained and available to work 1 shift per month in another clinic/department. Participates in a formal healthcare related education/presentation outside department. Completes mandatory trainings without supervision. Other: Excellent computer, interpersonal, and organizational skills required. Ability to problem solve and suggest improvements to work processes. Must perform work with high level of accuracy. Relationships Reports To: Director of Specialty Services. Other Contacts: Providers and their staff, patients and their families/visitors, hospital staff, public, and other health care/community agencies. Scope Machinery or Equipment Used: All office and patient care equipment. Physical Demands: Lifts, positions and/or transfers patients; pushes, moves and lifts heavy equipment/supplies; considerable reaching, stooping, bending, kneeling, crouching; prolonged extensive or considerable standing/walking; manual dexterity in using office equipment. Working Conditions: Exposure to crisis or hostile situations, infectious diseases/blood borne pathogens, and/or unpleasant elements of acute injuries from traumatic accidents as well as working with family members in their time of stress. Possible stress from emergency situation and multiple interruptions. Required Protective Equipment: PPE per policy as patient situation and condition dictates. Essential Functions Incorporates data collection and appropriate intervention skills in all interactions and delivery of nursing care including safety & protection, equipment use and management. Communicates with patients, team members, and management in an accurate and timely manner evidenced by talks with and listens to patients/team members, reports pertinent concise information, completes timely clinical documentation/order entry, protects confidentiality of patients/colleagues, applies critical thinking skills and considers alternatives. Organizes multitask workload, prioritizes tasks/work, seeks assistance, collects/integrates data from multiple sources, organizes/delivers care for multiple patient assignment, develops and maintains human care relationships, integrates the influence of the human experience of health, illness and/or healing, adapts care management for diverse populations and environment, advocates for needs of patients, provides care in ethical and legal manner, refers concerns/issues to appropriate resources, manages self and resources effectively, acknowledges limits of experience and capability, identifies learning needs and seeks resources. Utilizes equipment and supplies appropriately, maintains clean, orderly environment of care, ensures traffic control in “sensitive areas”, seeks out and accepts feedback, requests assistance appropriately, applies leadership skills, works within scope of practice, interacts with others in professional manner, participates in care within multi-disciplinary team, incorporates teaching in delivery of care with a focus on health promotion, provides appropriate information to patient, assesses for barriers to learning, uses colleagues as a learning resource, provides counseling about health behaviors, supports the preceptorship program, job shadows and student rotations, applies knowledge integration skills for the patient population served within unit, provides age specific care, applies knowledge of expected norms in planning care, applies new data to improve care, integrates technical, psychological, social, intellectual and physical skills, demonstrates the unit specific skills requisite for unit/patient population served, provides direct nursing care to assigned patients, sees unit specific competencies, serves as an active preceptor/mentor and/or super user for employees, participation in two or more process improvement opportunities within the last 12 months, proficient to cover other roles throughout your department, cross trained and available to work 1 shift per month in another clinic/department, participates in a formal healthcare related education/presentation outside your department, completes mandatory trainings without supervision. #J-18808-Ljbffr
