Saint Joseph's Medical Center is seeking a School Health Nurse Practitioner to manage care of school based program students including yearly physical examinations and updating of vaccinations. Perform routine assessments, anticipatory guidance and evaluation of chronic conditions. Develop, implement, coordinate initial and ongoing plan for patient care. Prescribe and dispense medications according to protocols. Work collaboratively with other health care professionals, school staff and community resources to ensure continuity of care. Participates in Quality activities / reporting and helps to develop projects / programs within the domain of the School Health Program. Requirements Pediatric / child / adolescent experience in a school setting is preferred. PNP / FNP License in NYS required. Bilingual Spanish / English preferred.
