The Palliative Care Nurse Liaison will serve as a member of the Palliative Medicine Service. The RN will assist in providing holistic compassionate care for seriously ill patients and their families.

Experience - Minimum of 3 years clinical nursing experience: palliative care, oncology, hospice, ICU and/or Geriatrics.

Education - Graduate of accredited school of nursing

Training - Completion of Respecting Choices Education within 1 year of hiring. Completion of any additional designated palliative care training as requested by director within allotted time frame.

Our healthcare team is working together to heal this community one patient at a time

* Collaborate with primary care physician regarding patient plan of care.
* Assist in alleviating psychosocial, emotional, spiritual, physical, and financial suffering.
* Assist in patient and family goals and plans of care.
* Act as a liaison for the Palliative Medicine Service within the hospital and the community
* Collect and analyze statistical/demographic data related to program needs.


* Patient Care
* Demonstrates critical thinking, analysis and clinical judgement through use of the nursing process to address the physical, emotional, psychosocial, and spiritual needs of the patients and families.
* Incorporates ethical principles and professional standards in the care of patients and families experiencing life-limiting, progressive illnesses as well as identifying and advocating for the wishes and preferences of patients and families.
* Recognizes dynamic changes in population demographics, health care economics, and service delivery that necessitate improved professional preparation for end-of-life care.
* Promotes the provision of comfort care to the dying as an integral component of nursing care.
* Communicates effectively and compassionately with patient, family, and health care team members about end-of-life issues.
* Recognizes ones own attitudes, feelings, values, and expectations about death and the individual, cultural, and spiritual diversity existing in these beliefs and customs.
* Demonstrates respect for the patients views and wishes during aggressive curative treatment and end-of-life care.
* Collaborates with interdisciplinary team members while implementing the nursing role in care.
* Uses data from symptom assessment to plan and intervene in symptom management using state-of-art, traditional, and complimentary approaches.
* Participates in planning for post-hospital care.
* Demonstrates the use of effective verbal, non-verbal, and written communication with patients, families, and members of the healthcare team in order to therapeutically address and accurately convey the palliative care needs of patients and families.


* Consistently performs 12 organizational Service Standards focused on Values, Service and Quality.
