Hospice Care Advanced Practitioner Northern Light Home Care and Hospice in Bangor, Maine, is seeking a full-time, compassionate, and skilled nurse practitioner or physician associate to join our dedicated hospice care team. In this rewarding role, you will review hospice eligibility, assess patient charts, and provide clinical guidance to nursing staff who perform community-based home visits. While this position is primarily remote, there may be occasional opportunities for face-to-face visits in the greater Bangor and Ellsworth regions. You will use your clinical expertise and heartfelt dedication to provide high-quality care for patients and their families, making a meaningful difference in their lives during times of greatest need. We continue with public health nursing by offering immunization clinics, adult health clinics, and education and awareness events for all ages. Throughout the homecare system, our clinicians make more than 160,000 in-home visits each year caring for an average of 1,400 people on any given day. An additional 4,000 individuals are cared for in health and wellness clinics.
