Education and Experience High school diploma or GED; and minimum of one to three months related experience and/or training; or equivalent combination of education and experience. Previous experience in a health care setting is preferred, but not required. Must provide certificate showing completion of 75 hour home health aide curriculum. Certifications, Licenses, and Other Special Requirements In accordance with state law, may need to possess current state certifications and follow regulations to maintain current certification. You will work closely with physicians, social workers, paramedics, and care coordinators to ensure each patient receives thoughtful, personalized care. This is a full‑time role with a schedule of Monday through Friday, no weekend work required. Key Responsibilities Deliver primary and palliative care to elderly patients in their homes (3‑6 patients per day). Perform physical assessments, develop care plans, and manage chronic and acute conditions. Provide education and counseling to patients and families. Manage medications, labs, imaging orders, and referrals as needed. Participate in interdisciplinary team meetings and care plan reviews.
