Physical Therapist - Home Care Location: Brooklyn, NY Employment Type: Full Time / Fee for Service Compensation: $15,000 sign on bonus for full‑time PTs $110,000 to $120,000 annual salary. Physical Therapist – Home Care at Cobble Hill LifeCares, a certified home health agency operating 24/7. We provide reliable, dignified care in the client’s home, specializing in physical therapy, personal care, and support services. Responsibilities Evaluate client’s level of physical functioning in the home and design a treatment plan, including modalities, frequency, expected outcomes, and short‑ and long‑term goals. Implement the treatment plan under physician direction and collaborate with agency personnel. Document treatment progress, report observations, and any changes to physician and program personnel. Collaborate with other professionals to establish rehabilitation and maintenance goals. Required Qualifications Licensed and registered Physical Therapist in New York State. Graduate of a PT curriculum approved by the American Physical Therapy Association or the Council on Medical Education of the American Medical Association. Minimum 1 year experience in a hospital or long‑term care facility as a PT. Experience working with a pediatric population (required); experience with adult and geriatric populations a plus. Knowledge of Medicare/Medicaid, regulatory and credentialing criteria and standards. Prior home‑care PT experience a plus. Reliable transportation for home visits in the assigned service area.
