_*$2000 Bonus Eligible Position for Full-Time*_

*JOB DUTIES*
* Assess home health pediatric patient for rehabilitative and functional status.
* Regularly re-evaluate the patient’s/client’s physical therapy need and evaluate the outcomes of care on a regular basis.
* Physical Therapist develops, initiates, and revises the plan of care as necessary to ensure quality and continuity of care.
* Identifies deficits including but not limited to muscle strength / coordination, mobility, gait, ROM and potential for intervention. Evaluates the home environment.
* Physical Therapist evaluates the need for and recommends DME devices and aides.
* Uses infection control measures that protect both the staff and the patient (OSHA).
* Coordinates services. Informs the physician and other personnel of changes in the patient's/client’s condition and needs.
* Physical Therapist demonstrates commitment and professional growth by participating in in-service programs and maintaining or improving competency.
* Supervises, teaches, and provides clinical direction to other physical therapy personnel as required or requested.
* Promote Agency philosophy and administrative policies to ensure quality of care.
* Physical Therapist provides effective communication to patients/clients, their family members, team members, and other health care professionals.

*QUALIFICATIONS*
* Graduate of an accredited Diploma, with Bachelor’s, Master’s, or Doctorate degree
* One year of experience as a Physical Therapist in a pediatric clinical care setting preferred
* Pediatric Home health experience preferred
* Reliable transportation
* Current Texas State license as a Physical Therapist

Pay: Up to $118,000.00 per year
