Job Summary

The Case Manager is a critical role within our hospital, responsible for coordinating the efficient and effective operations of case management and utilization review.

Key Responsibilities

  • Timely discharge planning to ensure seamless transitions for patients
  • Meeting Length of Stay (LOS) benchmarks to optimize resource utilization
  • Building and maintaining positive working relationships with physicians, payers, and hospital staff
  • Collaborating with transferring and receiving facilities to ensure smooth transitions

Requirements

  • Bachelor's degree in nursing preferred
  • Current South Dakota registered nurse license or multi-state privilege to practice in SD
  • BLS Certification

Work Schedule

  • Varied eight-hour shifts, including every third Saturday
  • PRN (as needed) schedule

Compensation

The hourly range for this position is $44.25, with actual hourly rate dependent on experience.
