Role Summary:

We are a provider-led, primary and post-acute care organization dedicated to redefining healthcare delivery for senior living communities. You'll collaborate with a multi-disciplinary care team to build quality patient care.

Qualifications:
  • MD/DO degree from an accredited medical school
  • Board Certified in Internal or Family Medicine
  • Current unrestricted license to practice medicine in the state of employment
  • Proficiency with EMR
  • Compassionate, intelligent, motivated, and a team player
