Schedule: Full Time Shift: Full Time Mon-Friday with Occasional weekends Location: Tampa Office The role you’ll contribute The Home Health Physical Therapist (PT) Care Manager is a professional therapist who coordinates and directs the home care patient’s services based on individual patient needs. The PT Care Manager is responsible for the independent management of the Home Health patient population requiring the use of advanced assessment, teaching, and decision-making skills. The PT ensures that appropriate referrals to other services are made, interdisciplinary conferencing takes place regularly, and appropriate documentation is completed. Relevant knowledge and experience are consistently applied to new patient populations. The PT Care Manager cares for a caseload of home health patients requiring therapy as the primary service by evaluating the patient for appropriateness of home health and developing the home care plan in conjunction with the physician. S/he educates patients, families, caregivers, and community providers to safely perform care. S/he provides follow-up by evaluating the effectiveness of the home care plan and monitoring the patient/family’s response to the plan to achieve patient/family goals and top decile outcomes. The PT Care Manager also identifies performance improvement and home health standard of care initiatives and assists in designing or implementing programs to address needed changes. Develops patient-specific care plans with realistic, measurable goals in consultation with patients, families, and physicians that considers the physical, cultural, psychosocial, spiritual, age-specific, and educational needs of the patient. Reassesses patients at appropriate intervals, documents progress, and determines the effectiveness of therapy. Observes and records patient activities and findings, reporting any changes or deviations in the plan of care to the physician. Instructs patients and caregivers on the use of assistive devices and establishes home programs. Documents treatments accurately and timely in medical records and updates care plans as necessary. The expertise and experiences you’ll need to succeed Minimum qualifications: Bachelor's in Physical Therapy Physical Therapist (PT) Active American Heart Association BLS 1+ year relevant clinical physical therapy Preferred qualifications: Doctorate in Physical Therapy Valid in state Driver’s License with current car insurance Cert for Oasis Specialist-Clinical (COSC) Home Health Care-Manager Certification #J-18808-Ljbffr
