POSITION SUMMARY:

This is a full time home clinic position (30-35 hours/week) supporting our clinic in Wilmington (Market St.). Schedule is 2-3 weekdays plus every other weekend.

CVS Health is seeking a Nurse Practitioner (NP) to join our Primary Care team. In this capacity, you will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients. This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning. This role will report to the practice manager.

PRIMARY DUTIES AND RESPONSIBILITIES:

Responsibilities include but are not limited to

Patient-Centered Quality and Safety

Uses knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions relevant to primary, acute, chronic, and transitional to provide care to patients Performs history and physical assessments of patients that are accurate, complete, concise, relevant, and appropriate for age, gender, and clinical symptoms. Provides patient counseling; inclusive of pregnancy prevention, STI Prevention/safer sex practices, contraceptive care counseling and medication management Counsels and educates patients and families on health maintenance and respond to patient care inquiries Evaluates and treats patients' medical needs with safe and high-quality treatment Documents all patient care within an electronic health record in accordance with current policies and procedures Works as a member of the clinical care team providing care and coordination of our patients with internal and external colleagues ensuring the highest level of care is always provided for all patients Provides patient-centered care that is compassionate, appropriate, and effective for the promotion of health, prevention of illness and treatment of disease Refers to appropriate specialists and patient care components as appropriate Maintain patient confidentiality in accordance with PHI and HIPAA standards Communication and Interdisciplinary Collaboration

Completion of master's degree level Family Nurse Practitioner with current National Board Certification and State of Employment license to practice in the Advanced Practice role required.

Required Experience:

A minimum of 1 year experience as a Nurse Practitioner is required. Experience in Family medicine or primary care practice preferred. Internal applicants with less than 1 year experience are eligible with successful completion of the CVS Minute Clinic Primary Care Training Program Active Advance Practice Registered Nurse, Nurse Practitioner license in good standing Current National Board Certification in Family Practice ADDITIONAL REQUIRED SKILLS AND ABILITIES:

Working knowledge and understanding of quality measurement in the management of chronic disease conditions. Self- motivated, prioritizes and solves problems, takes initiative, and advocates for their patients and their practice Effective verbal, written, and electronic communication skills Outstanding organizational skills and ability to multi-task Initiative, problem solving ability, adaptability, and flexibility Ability to work remotely in a clinical care team culture Ability to work without direct supervision and practice autonomously Is proficient with information management and technology Willingness to obtain multi-state licensures Capacity to collaborate with professional colleagues frequently to ensure quality patient care WORKING ENVIRONMENT:

While performing the duties of the job, the Primary Care NP is regularly required to interact with patients remotely, sit, write, operate the computer and phone, speak intelligibly, and hear patient responses Specific vision abilities include the ability to view and read a computer screen and other electronic devices

Pay Range

The typical pay range for this role is:

$37.66 - $81.11 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
