Job Summary:
TriStar Centennial Medical Center s premier cardiovascular consultant group is seeking a certified acute care provider for a VAD coordinator role in Nashville, Tennessee.Qualified Candidates:
- Acute care nurse practitioner or physician assistant certification
- Tennessee licensure or ability to obtain
- Two years hospital experience in a cardiac or CV surgical ICU
- Experience caring for mechanical circulatory support and advanced heart failure patients is preferred
- Full time, 4 10-hour shifts
- Shared call 1:3 with other coordinators
Job Responsibilities:
- Coordinates and participates in patient evaluation and selection for advanced therapies
- Coordinates/leads patient selection and performance improvement meetings
- Outpatient clinic visits, clinical management, and care coordination (shared call with 2 other coordinators)
- Inpatient discharge planning/care coordination
- Inpatient and outpatient education to VAD patients and their caregivers
- Oversees/provides education to acute care staff and community resources (EMS, rehab, home health, etc.)
- Participates in the development and maintenance of VAD statistical databases and associated registries (INTERMACS)
- Equipment and supply management using online inventory tool
- Joint Commission certification preparedness
- Involvement with community outreach on VAD safety measures and awareness
- Liaison between surgeons, heart failure cardiologists, industry partners and patients
