At Residential Home Health and Hospice, we're committed to delivering exceptional patient care. As a Home Health Medical Social Worker, you'll collaborate with our experienced team to develop and implement personalized care plans that address the unique needs of our patients.

Key Responsibilities:

  • Develop and manage patient care plans in collaboration with our multidisciplinary team
  • Conduct thorough assessments of patient needs and create comprehensive care plans
  • Provide emotional support and counseling to patients and their families
  • Communicate effectively with patients, families, and healthcare team members

Requirements:

  • Master's degree in Social Work from a graduate school accredited by the Council of Social Work
  • One to two years of post-graduate experience in a healthcare setting
  • Current Medical Social Worker license issued by the state in which you work
  • Ability to spend approximately 20% of your day driving to and from patient locations
  • Strong time management and organizational skills
  • Proficiency in using tablets, mobile phones, and EMR software
