Position Summary Collaborates with the interdisciplinary team to coordinate the overall patient care for anyone referred to Hunterdon Health#with a known or suspected diagnosis of cancer, facilitating a rapid, efficient transition through the system from referral to diagnosis and treatment; Provides patient education regarding diagnosis and treatment plan; facilitates referrals to appropriate services within and outside Hunterdon Health#to meet the physical, psychosocial, financial and spiritual needs of patients. Primary Position Responsibilities Facilitates and Coordinates Patient-centered care, follow patients through their initial course of care through multiple areas (medical oncology, radiation, surgery, hospital) Serves as patient and family advocate, providing education and addressing barriers to care Demonstrates effective resource management and interprofessional collaboration Participates#in and contributes expertise to activities and functions that strengthen the cancer program (including, but not limited to continuous program and quality improvement, relationship building and cancer program marketing strategies, plans, develops, and implements patient centered programs as part of disease management team, participating in community screening and community cancer awareness activities, fundraising events, etc.) Actively seeks and participates in educational opportunities, both formal and informal, to achieve and maintain knowledge and skill related to job responsibilities. Adheres to and actively participates with Commission on Cancer and the National Accreditation Program of Breast Center standards (including but not limited to data collection and analysis, continuing education, protocol completion and review, survivorship, etc.) Qualifications Minimum Education: Required: All hires after July 2012 are required to have a Bachelor#s Degree in nursing OR if Associate#s Degree or Diploma, RN must provide proof of enrollment in a BSN program prior to hire, be continuously enrolled and complete within 5 years of hire date or internal RN promotion date. Preferred: Bachelor#s Degree in Nursing strongly preferred Minimum Years of Experience: Required: Three (3) years experience as a registered nurse. Preferred: Five (5) years experience as a registered nurse At least one year recent acute care experience or experience in a healthcare system Recent experience as a care coordinator Oncology experience is strongly preferred License, Registry or Certification: Required: NJ state license in good standing and American Heart Association CPR certification Certification in oncology (examples include but not limited to: Oncology Certified Nurse, Certified Breast Care Nurse or Oncology Patient Navigator-Certified Generalist) OR complete a qualifying patient navigation training program within one year of hire. Preferred: Certified Oncology Nurse Knowledge, Skills and/or Abilities: Required: Excellent organization and communication skills (written and verbal); strong computer skills, including Google Workspace. Self-directed; strong team player, collaborative; calm under stress; resourceful. Commitment to continuous quality improvement and life-long learning.
