By partnering and working with consumers we assist in enabling them to safely live in their own home and community.The successfulapplicant will be someone who can independently manage and grow a portfolio of Support at Home clients.Key Responsibilities include:Partnering with consumers to develop Support at Home care plansOngoing case management of transitionedHome Care Package consumersProactively promote White Oak to potential consumers and community referral partners (GPs, Social workers, Assessment Agencies, Hospitals, Pharmacies etc.)Regular monitoring and monthly reporting on consumer budgetsWorking closely with White Oak clinical teamScheduling appointments for assessments and reviewsWorking with other program and rostering managers to ensure a consistently high level of service is deliveredKeeping up-to-date with government legislated standards, guidelines and reformsDesired Skills and ExperienceTo be successful in the role, you will be a well-presented, highly empathetic and compassionate person who prides themselves on delivering exceptional customer service. In addition, you will ideally possess:Previous experience in Home Care Package coordination (now Support at Home)Highly developed communication and organisational skillsAbility to work in a dynamic, fast-paced and changing environmentHigh level computer literacy (MS Office applications and CMS; preferably Alayacare)Kind, caring and respectful of consumers and colleagues identity, culture and diversity.Full-time AvailabilityNDIS Worker Screening Check (or ability to obtain) #J-18808-Ljbffr
