What You'll Do:
Foster Team Collaboration: Work closely with the Access Team and other healthcare professionals to ensure a cohesive and comprehensive approach to patient care.Welcome New Patients: Conduct thorough initial assessments and collaborate with the Interdisciplinary Team (IDT) to develop individualized care plans ensuring a smooth transition into our care program.Educate and Support: Provide essential education to patients and their families about disease management, care options, and what to expect, guiding them through their care journey with confidence.Document with Precision: Maintain accurate and detailed documentation of patient information, ensuring all records are up-to-date and reflective of the care provided.
