p>As a Family Nurse Practitioner at SSM Health in Richmond Heights, Missouri, you will play a vital role in delivering comprehensive primary care services to patients of all ages within a collaborative healthcare team. You will provide personalized, evidence-based care focused on promoting health and preventing disease in our community.

Patient Assessment and Management: Conduct thorough assessments, including medical history reviews and physical examinations, to diagnose and treat acute and chronic health conditions in patients of all ages.

Primary Care Services: Provide a full spectrum of primary care services, including health promotion, disease prevention, routine screenings, vaccinations, and management of common illnesses and injuries.

Chronic Disease Management: Develop individualized treatment plans for patients with chronic diseases such as diabetes, hypertension, asthma, and COPD. p>

Patient Education: Empower patients and families by providing comprehensive education on health maintenance, disease prevention, and treatment options. Address questions, concerns, and health-related inquiries to promote informed decision-making and adherence to care plans.

Collaborative Care: Collaborate with physicians, advanced practice providers, nurses, and allied health professionals to coordinate patient care across multiple disciplines and healthcare settings. Facilitate referrals to specialists and other healthcare providers as needed.

Electronic Health Records (EHR) Documentation: Maintain accurate and timely documentation of patient encounters, treatment plans, and follow-up care using electronic health record systems in compliance with regulatory standards and organizational policies.

Quality Improvement: Participate in quality improvement initiatives and clinical audits to enhance the delivery of primary care services, optimize patient outcomes, and ensure adherence to clinical practice guidelines.

Qualifications:

  • Master's degree in Nursing from an accredited program
  • Current licensure as a Nurse Practitioner in Missouri with national certification as a Family Nurse Practitioner (FNP)
  • Strong clinical assessment, diagnostic, and treatment skills
  • Excellent communication and interpersonal skills
  • Ability to work independently and collaboratively within a multidisciplinary team
