Speech Language Pathologist (SLP) – In House Full Time Evolve Therapy Services, LLC – therapy management organization – is hiring a compassionate Speech Language Pathologist for an in‑house position at a skilled nursing facility located in Columbus, NE. Job Purpose Assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with applicable federal, state, and local standards, guidelines, and regulations to ensure the highest quality resident care. Job Functions Provide quality speech therapy services and initiate referrals when appropriate, according to standards of practice, department procedures, and productivity standards. Evaluate and obtain data necessary for treatment planning and implementation. Develop a treatment plan that includes a target list, short‑term, and long‑term goals. Instruct patient, family, and staff in appropriate speech‑language procedures to promote carry‑over and goal achievement in daily activities. Complete compliant and medically necessary documentation in accordance with CMS expectations in NetHealth EMR systems. Oversee and implement proper protocols for FEES/MBS with coordination of site programs. Participate in screening protocols to ensure residents’ needs are met timely and with clinical excellence. Education & Qualifications Master’s degree in Speech Language Pathology. Certificate of Clinical Competency. Active SLP license in the state(s) of practice.
