**Description:**

Per diem - VRN at Albany Memorial Hospital HUB

As a Virtual Registered Nurse (RN) on the care team you will be a part of/responsible for:

+ Collaborating, communication & facilitation of optimal provision of care through a computerized &/or digital platform supporting daily management of care.

+ Responsible for the assessment, planning, implementation, coordination, monitoring & evaluation of the patient's plan of care from admission to discharge.

+ Utilize your clinical knowledge, critical thinking skills & the principles of case management & adult education to coordinate, implement & support the plan of care.

+ Ensure a seamless, effective, efficient transition of care across the continuum.

+ Monitoring quality metrics specific to the department & ensuring complete & accurate documentation in the patient record.

+ Perform the roles of mentor, clinician, educator, consultant/collaborator, and navigator.

Virtual RN Requirements:

+ -New York State Valid Nursing Licenses from an accredited professional School of Nursing.

+ Minimum of 8 years in healthcare/critical care as a Registered Nurse

+ Experience with Electronic Medical Records

+ Current or recent bedside RN experience

Virtual RN Preferred Requirements:

- BSN

Pay Range: $44.00 - $51.95

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
