Cardiology Outpatient Advanced Practitioner opportunity in the Hartford CT region.Trinity Health Of New England seeks a Nurse Practitioner or Physician Assistant to join the Cardiology Heart Failure team at Saint Francis Hospital and Medical Center.RESPONSIBILITIES: See outpatients in various Heart Failure Centers throughout the region including St. Francis Hospital and Medical Center in Hartford. One site per day. Training begins at SFHMC.Provide transitional care managementOffer guideline directed medical therapy optimizationPerform advanced heart failure evaluationsProvide same day access care The successful candidate will: Interested in heart failure careHave Cardiology/Heart Failure experience (preferred but not required)Hold an Advanced Practice Nurse or Physician Assistant CT state license.Enjoy a competitive compensation and benefits package including paid time off and 403b matching.
