Home Health Licensed Practical Nurse Opportunity

We are seeking a skilled and compassionate Home Health Licensed Practical Nurse to join our team at Brookdale Home Health St. Louis. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

About the Role

The Licensed Practical Nurse (LPN) in Home Health is responsible for providing high-quality patient care and performing technical skilled care in the patient's home. This role requires a strong commitment to delivering exceptional patient outcomes and a passion for working in a dynamic and supportive team environment.

Responsibilities
  • Provide services within the scope of practice as defined by state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the healthcare team.
  • Instruct patients and their families in the care of the patient and maintenance of a healthy environment for the patient.
  • Observe, record, and report to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition daily.
Requirements
  • Current LPN licensure in the state of practice.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR Certification required.
