Hospice RN Case Manager

We are seeking a compassionate and skilled Registered Nurse to join our Hospice team as a RN Case Manager. As a RN Case Manager, you will play a vital role in delivering high-quality end-of-life care to our patients and their families.

Responsibilities:
- Conduct initial nursing evaluations to determine eligibility for hospice services.
- Identify patients' physical, psychosocial, and environmental needs and re-assess as needed.
- Develop and implement individualized plans of care while maintaining accurate and relevant clinical notes.
- Collaborate with patients, families, attending physicians, and other members of the IDG to provide comprehensive care.
Requirements:
- One year of clinical experience as a Registered Nurse.
- Current RN licensure in the state of practice.
- Current CPR Certification.
- Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
Additional Requirements:
- At least one year full-time experience as an RN in a healthcare setting.
