Job Title: Registered Nurse / Back RN/LPN - Family Medicine - East Mason, Green Bay, WILocation: East Mason, Wisconsin, United StatesSummary: The Registered Nurse delivers patient-centered care, triages calls, assesses needs, and facilitates appointments with physicians or advanced practice providers. May provide direct patient care as needed and supports high-quality, safe care within scope of practice. Responsibilities Manage patient and family health concerns through telephone triage and consultation. Assess patient circumstances, problems, expectations, and needs; facilitate access to appropriate level of care. Refer to healthcare providers, services, and community resources as needed. Collaborate with other healthcare providers to develop and implement patient care plans. Conduct follow-up calls, provide test results, and offer health counseling and education. Provide direct patient care, including patient rooming, collecting medical information, and performing initial assessments. Assist physicians or advanced practice providers with procedures and arrange necessary follow-through. Administer medications as within scope and communicate care plans to patients and staff. What You Will Bring Active nursing license (RN or LPN) in Wisconsin. Experience in family medicine or primary care is preferred. Ability to work in a collaborative, patient-centered team environment.
