Case Manager, Brownsville Homebase Program RiseBoro Community Partnership – 1875 Broadway, Brooklyn, NY Full‑time, Day Shift, $27.19–$31.40 per hour. Monday‑Friday, 9AM–5PM. Join RiseBoro’s Homebase Program, NYC’s premier homelessness prevention initiative, dedicated to helping households at risk of shelter entry achieve long‑term housing stability. Essential Duties and Responsibilities Coordinate services and activities to provide participants with tools to sustain housing and prevent homelessness. Work with clients, household members, supervisors, and supplementary staff to execute individual service plans. Establish goals and actions addressing immediate housing crises and underlying causes. Monitor and evaluate participants’ progress in implementing service plans. Confer client cases with the Supervisor and Program Director to determine necessity of financial subsidies. Prepare necessary documentation for in‑house subsidy applications on behalf of clients. Provide referrals to community supports, public benefits, and entitlements. Maintain accurate record‑keeping via case files and the designated computer database. Engage in arrangement, coordination, monitoring, and delivery of services to program participants. Ensure participants’ rights are protected; perform other duties as assigned by Supervisors or Program Director. Core Competencies Bilingual English/Spanish preferred. Experience in nonprofit organizations and a commitment to social justice. Excellent communication and advocacy skills. Ability to work independently and collaborate with a team. Entrepreneurial spirit, problem‑solving ability, and relationship‑building skills. Education and Experience Associate degree required with three years of experience; Bachelor’s degree preferred.
