DescriptionCommunity Options, Inc. is a national non-profit agency providing residential group home, day habilitation and supported employment services to individuals with disabilities in 12 states.We are seeking aLicensed Practical Nurse (LPN)inMercer County, NJ.The LPN delivers care to individuals in a community setting utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. This person will interact with individuals, significant others, and health team members while maintaining the standards of professional nursing.Starting pay is $24.00/hourResponsibilitiesProvide treatment and health care procedures ordered by a physician and required by the standards of professional practice or lawEnsure individuals attend medical appointments per their Individual Support Plan (ISP)Advise supervisor of all concerns and issues related to the provision of nursing servicesCreate and distribute medical/health reports detailing general health, health care goals, and physician recommendationsDevelop and update Individual Support Plans (ISP)/Person Centered Plans (PCP) with an interdisciplinary team to include any concerns, observations, and behavioral developmentsTrain employees on the medical needs of assigned individualsMonitor health and medical conditions of individuals and refer for medical services as neededMonitor the use of medications through medication administration logsEnsure health service supplies are available and replenished as neededEnsure program documentation and billable records are completed accurately and timelyBuild a support network for individuals by developing and maintaining relationships with community members, businesses, and familiesParticipate with individuals in community, social, and recreational activitiesParticipate in circles of support for personal futures planningFollow strict OSHA procedures regarding blood borne pathogensEmployees must cooperate with the licensee and department staff in any inspection, inquiry or investigationAdditional tasks and responsibilities may be assignedMinimum RequirementsBachelor’s Degree or equivalent experience in nursing fieldMinimum of one year of experience providing nursing services to individuals with disabilitiesValid nursing licenseValid driver’s license with a satisfactory driving recordComplete all state and agency required training per state guidelinesAbility to calculate figures related to medication dosagesUnderstanding and commitment to community-based support for persons with disabilitiesAbility to exercise good judgment and remain calm in crisis situationsAbility to establish a comfortable and supportive relationship with an individual receiving supportsProficient with Microsoft OfficeExcellent verbal and written communication skillsAbility to effectively present facts and recommendationsAbility to effectively communicate with program participants, third-party agencies, staff and members of the communityExcellent time management skillsWorking ConditionsWork in office, residential programs, day programs, and community environmentsMay be exposed to changing weather conditions when working outsideHours and weekly schedules may change due to business needs and may include evening and weekend hoursMay be required to be on-call in cooperation with other management staffOccasional overtime may be requiredFrequent lifting, stretching, and other physical exertion may be requiredMay be required to lift or move 50+ poundsMay occasionally assist with wheelchair transfer of non-ambulatory individualsMay be required to transport individuals utilizing your own vehicle or company provided vehiclesMay be exposed to various medical conditions, communicable diseases, blood, body fluids, or tissuesWhy Community Options?Competitive Insurance Benefits (Medical, Dental, Vision)Paid Holidays—Including a Birthday HolidayGenerous PTOEmployee Incentive & Discount Programs403b Retirement PlanIncredible career growth opportunitiesCommunity Options is an Equal Opportunity EmployerM/F/D/V
