Avera is currently seeking Registered Nurses (RN) looking to deliver the best possible nursing care in Avera’s patient-focused settings. *This position may be eligible for a $6,000 sign on bonus* Position Highlights: Student Loan Repayment: This RN position may be eligible for Avera’s Student Loan Repayment Program, which helps you pay off your loans and save money. Each month an extra payment of $167 (up to $10,000) from Avera will help pay down your principal balance faster. Job Summary: The Case Manager is responsible for the care planning, utilization of services, coordination of care, and the education of the interdisciplinary teams in the management of patient care services. The focus of patient case management is to provide patient centered care efficiently, effectively, and with the best possible outcomes. The Case Manager will develop all patient care plans in collaboration with the physician and interdisciplinary team. Work Schedule: Monday-Friday 8:00am-5:00pm, with wknd/hol/call involvement; 72hrs/2wks Weekend rotation: every 5th at this time Holiday rotation: every 5th holiday at this time Call time involvement: average about 3-4 call shifts a month Pay: The hourly range for this position is listed below.
