Independent Prescribing Clinical Pharmacist Reach Healthcare is a large clinically-led GP practice and its own Primary care network, patient-centred primary care organisation serving communities across Medway. With a strong focus on holistic, responsive, and excellent care, we operate across multiple sites committed to innovation, teamwork, and improving patient outcomes. We have a fantastic team consisting of GPs, Allied Health Professionals, Practice Nurses and a wealth of experienced administrative staff who support the clinical team. We are currently seeking an Independent Prescribing Pharmacist to join our friendly, multidisciplinary team. This is a fantastic opportunity to contribute directly to patient care, leading on safe and effective prescribing, managing chronic conditions, and supporting medication optimisation across our practices. What You’ll Do Conduct patient consultations and prescribe within your clinical competence Perform medication reviews and support deprescribing initiatives Collaborate closely with GPs, AHPs, nurses, and other professionals to enhance care pathways Take a proactive role in managing long‑term conditions and promoting antimicrobial stewardship Provide training and guidance to colleagues on medicines‑related best practice What We’re Looking For GPhC-registered Pharmacist Independent Prescriber qualification Experience in a GP or primary care setting Excellent clinical judgement, communication, and consultation skills Passion for holistic, patient‑focused care Desirable: Experience with EMIS, chronic disease management, and audit participation.
