Description The Clinical Pharmacy Liaison is integral to supporting the clinical team in ensuring the safe, effective, and timely administration of medication for patients managing diabetes and associated conditions. Working under the supervision of the Director of Specialty Pharmacy Services and in collaboration with diabetes educators, providers, and care teams, the liaison plays a critical role in assisting patients with insulin, oral diabetes medications, continuous glucose monitors (CGMs), insulin pumps, and other diabetes-related therapies, ensuring patients have timely access to the medications and supplies they need as part of their hospital-based diabetes care plan. This position demands a strong attention to detail, proficiency in diabetes medications and supplies, and a patient-centered approach to healthcare. Qualifications Minimum Education High School Diploma or GED (Required) Minimum Work Experience 2 years Relevant pharmacy work experience (Required) Required Skills/Knowledge Knowledge of diabetes medications, devices, and current standards of care. Familiarity with insurance billing, prior authorization processes, and patient assistance programs. Excellent organizational, communication, and teamwork skills. Strong patient-centered focus with the ability to work in a fast-paced hospital environment. Excellent verbal and written English communication skills required; as well as ability to communicate professionally over the phone. Excellent interpersonal skills to provide superb, personalized customer service to instill confidence and advocate for patients; ability to explain required information to customers in a comprehensible manner. Cultural sensitivity, understanding, and comfort with a wide range of social, racial, and ethnic populations. Flexibility to adapt to changes in the departmental needs (i.e., assisting other team members, adjusting assignments, etc.). Ability to quickly learn other relevant applications that support management of patient care and assigned responsibilities; and ability to extract necessary information. Required Licenses and Certifications District of Columbia Pharmacy Technician License 30 Days (Required) Certified Pharmacy Technician (CPhT) Upon Hire (Required) Functional Accountabilities Coordinate Medication Ordering and Delivery Manage the entire medication ordering and delivery process from start to finish. This includes verifying medication orders, ensuring timely procurement, and collaborating with local pharmacies to secure medication availability. Work closely with the inpatient and outpatient teams to ensure medications are delivered promptly and accurately to meet patient care needs. Ensure Accurate Documentation Ensure that all medication-related activities, including ordering, delivery, and patient receipt of medications, are properly documented in the patient's medical records. This includes charting medication and supply updates, tracking inventory, and ensuring compliance with all regulatory and institutional requirements throughout the process. Partner with Local Pharmacies to Ensure Medication Availability Work closely with local pharmacies to coordinate medication availability, ensuring that patients' prescriptions are filled and ready for timely administration. This partnership helps prevent medication delays and ensures patients receive the necessary treatments without interruption. Collaborate with Clinical Staff to Address Medication and Supply Request Partner with various clinical staff members, including physicians, nurses, and case managers, to address medication and supply requests. Ensure that medications, supplies, and any special requests are met in both inpatient and outpatient settings, prioritizing patient safety and care needs. Support Inpatient and Outpatient Medication Management Ensure that both inpatient and outpatient medication needs are met efficiently. In the inpatient setting, this includes managing patient-specific medication orders and collaborating with the healthcare team for timely delivery. In the outpatient setting, coordinate with providers to ensure that patients have access to medications and supplies for continued care after discharge. Monitor Medication Therapy and Compliance Assist in the monitoring of patients' medication therapy, ensuring adherence to prescribed regimens. Collaborate with the clinical team to address any concerns regarding medication compliance or potential delays in medication administration. Assist with Inventory Control and Medication Reconciliation Assist in medication reconciliation during patient transitions of care, ensuring that all medications are appropriately accounted for and continued in both inpatient and outpatient settings. Provide Support for Medication Education Support patient education efforts by ensuring that patients receive appropriate information on their medications and supplies. This includes ensuring that all educational materials are available and that patients have the necessary resources to manage their medications effectively post-discharge.
