Duties and Responsibilities:

Provide psychiatric evaluations and medication management for geriatric clients in an outpatient clinic setting, at a clinic location, via telehealth, or in the home, if needed
To participate with other staff members in the development of a treatment plan based on the diagnostic assessment, to periodically reassess patients' needs and circumstances and revise treatment plan if appropriate;
To help coordinate the patient's care with hospitals and other providers;
To help with hospitalization when appropriate;
To conduct seminars and play a part in the education of the team where time permits and the necessity arises;
To be available to the team for consultations, when necessary.
Attends and participates in staff meetings and periodic in-service trainings when available.
Additional tasks as assigned.

Requirements:

Valid NYS license to practice in discipline
Current DEA Registration Number
Possession of or ability to obtain a NPI number and Medicare Provider Number
Desire and ability to work collaboratively and engage in quality improvement activities as part of a committed, integrated team
Excellent interpersonal skills and the ability to communicate effectively and concisely both in writing and orally
Strong attention to detail with the ability to solve problems; to elicit, comprehend, and synthesize accurate and specific information; prioritize work; to work independently; and to excel in a high-performance culture
An understanding and appreciation of the roles of race, religion, ethnicity, sexual preference, and individual values as they relate to serving clients
Professional maturity and strong commitment to JASA's mission
Good writing/record keeping skills
Basic computer skills
Familiarity with electronic health records and e-prescribing desired
Ability to read, write speak and understand English
