Job Post Title: Full-Time FNP (or PA) at 5 th Street Medical Plaza in Tyler, TX Job Post Description: 5 th Street Medical Plaza seeks a Family Medicine Nurse Practitioner or Physician Assistant to join the fast-growing family of UT Health East Texas hospitals and clinics. About the Position Average patient load of 18-25 per day. Join a team of 7 Physicians and 1 APP. EMR: EPIC. M-F 8a to 5p. Outpatient only, no weekend requirement. Lab and X‑Ray on site.
