We are seeking a compassionate and skilled Home Health Registered Nurse to join our team at Jefferson Health at Home by BAYADA. As a Home Health RN, you will have the opportunity to provide high-quality patient care in the comfort of their own homes.
Key Responsibilities- Deliver skilled nursing care to patients in their homes, including administration of medication, wound care, and treatments.
- Conduct thorough assessments of patients' conditions and develop individualized care plans to meet their unique needs.
- Provide education and support to patients and their families to promote independence and self-care.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Active and current license to practice as a professional registered nurse (RN) in the state of PA.
- One year of clinical experience preferred, but not required.
- Ability to travel to patient homes as assigned.
- Strong assessment and communication skills, with a calm and reassuring presence.
- Flexible scheduling to accommodate your needs, with a variety of shift options available.
