This is a full-time (40 hours per week) position with week-end responsibility/rotation supporting our High Point, Kernersville and Winston-Salem service areas. Job Summary:Evaluate the clinical needs of newly referred and hospitalized hospice patients. Identify and inform patient and family about appropriate resources. Work with the patient/caregiver and appropriate providers to establish patient/caregiver care goals and identify and implement interventions. With attending physician and selected members of the IDT, ensure that a medical plan of care is established and implemented for the patient. Communicate effectively with other members of the interdisciplinary team and health care providers, as needed.Excellent assessment, clinical nursing, and coordination skills, and effective interpersonal and communications skills necessary.
