PRACTICE OVERVIEW
AdventHealth Well 65+ offers a multi-disciplinary approach to primary care for members 65 and older, dedicated to providing members whole person care for all health and well-being needs in one location.
Since its inception in 2021, AdventHealth Well 65+ has consistently stayed in the 95%+ range for patient satisfaction. strong> Includes RN Case Management, 2 Health Coaches (MA s) per provider, Social Worker, Pharmacy, Care Coordinators, Welcome Coordinator, HCC Coding Support, and Administrator. Work in a team-oriented environment with like-minded physicians in offices housing 4-6 providers. You will have the opportunity to practice medicine as it should be and utilize all of your clinical skills-preventive care, chronic disease management, acute care, procedures, resulting in lower number of appropriate specialty consults.
