Organizational Relationship:

Job Summary:

· The Physical Therapist provides evaluation and treatment to children identified by the school district as a child/student with a disability as outlined in Part 200 Regulations/IDEA

· The therapist utilizes principles of growth and development in establishing guidelines for the implementation of the child’s IEP to provide treatment as able

· The Physical Therapist provides services in conformance with all regulatory bodies governing this population, school policy and procedure and in compliance with the NYS Physical Therapy Act

Qualifications:

· Bachelor’s degree or higher in Physical Therapy from an institution accredited by the APTA

· New York State License/Registration

Job Responsibilities:

· Evaluate children with known or suspected disabilities as pertinent to case assigned (e.g. EV1P’s)

· Participate in planning/meeting with the school district, parents, and other members of the CPSE and CSE

· Provide treatment in accordance with the child’s IEP

· Provide all required documentation within required time frames including daily notes, quarterly progress notes, annual reviews, and notes in EZ Trac

· Provide instruction to families, team members and other support staff consistent with accepted practice standards

· Instruct staff and families in the use of, adaptive equipment and/or other assistive devices (e.g. appropriate seating)

· Track and monitor PT scripts. The therapist will notify the lead PT and the Administrative Assistant in the main office 3 months prior to the expiration of said script in order to obtain a new one

· Must maintain and provide the school with current malpractice insurance documentation

· Therapists will clean and sanitize materials, toys, equipment in his/her office after each session and during the day in between students

· Must provide make-up sessions when absent

· Must sign students in and out of classrooms

· Submit all required attendance documentation to administration within required time frames

· Maintain effective contact with other team members via weekly scheduled team meetings

· Include the family in planning and treatment of child

· Obtain NPI#

· Serve as a role model for student and staff in demonstrating positive attitudes, appropriate attire and grooming and effective work ethic

· Participate in school-wide projects/committees

· Protect confidentiality of records and information gained as part of exercising professional duties and use discretion in sharing such information within legal confines

· Notify school of absences due to illness, emergency leave, normal vacation periods, or professional meetings that will affect agreed upon services in the school

· Treat all children with dignity and respect

· Attend 3-5 evening orientation/conference events with parents

· Performs other duties assigned by Lead Therapist and Principal

· Must be able to lift 40 pounds

· Will ensure that every therapist will sanitize materials/equipment/toys in his/her office periodically and between students

· Will ensure that they will follow all the policies and procedures of Positive Beginnings, including the current Reopening Plan as well as the Federal, NYSED, NYC DOHMH guidelines/rules and regulations.

Job Type: Full-time

Pay: $65.45 - $70.09 per hour
