**Position Highlights:**
+ Practice with a multi-disciplinary team of heath care providers that includes Primary Care Physicians, RN Case Managers, social workers, and clinic staff
+ Our model is designed to provide the most efficient, comprehensive, and proven care techniques to treat the whole patient - physically, mentally, and socially - at each visit
+ Primarily manage care in the patient's home; however, care may require an office or other facility setting depending on the needs of the patient
+ Provide care for some of our sickest patients - patients whose needs are beyond what our care management and clinic-based resources can provide, but whom hospice services are not appropriate
+ Diagnose and treats both acute and chronic conditions such as diabetes, hypertension, kidney disease and other chronic medical conditions
+ Treat acute conditions such as minor injuries and other acute medical conditions such as infections or exacerbations of chronic diseases
+ Facilitates patient and family decision making regarding treatment decisions, end of life care, and serious illness discussions
+ Prescribes pharmacological and non-pharmacological interventions

**Compensation/Benefits Highlights:**
+ Base Salary plus Annual Quality Incentive with a target of $20,000
+ Optum Care Advanced Provider Clinician Partnership Plan

**Required Qualifications:**
+ Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA)
+ Current Licensure through the Texas Board of Nurse Examiners OR Licensed Physician Assistant by the Texas State Medical Board
+ Valid DEA license or the ability to obtain one prior to employment
+ 1+ year of clinical experience as a Nurse Practitioner or Physician Assistant
+ Comfortable managing a primarily Medicare-aged patient population
+ Reliable transportation
**Preferred Qualifications:**
+ 3+ years of clinical experience as a Nurse Practitioner of Physician Assistant
+ Home health experience
+ Bilingual (English/Spanish) fluency
+ A working understanding and support of a managed care environment
