Our skilled nursing facility located in Madison, Ohio provides an inviting, home-like environment conveniently located near I-90. This 120-bed facility has private rehabilitation suites with easy access to the therapy gym. The therapy staff and interdisciplinary team strive to create a comprehensive care plan that is unique to each short-term, outpatient, or long-term patient.

\*\* *Supervising Physical Therapist Part time or Full time available 10-29hrs a week*\*\*

Join our _*in house therapy team*_ at Heritage Rehab as a Physical Therapist dedicated to providing exceptional care. We are looking for a passionate individual with expertise and a commitment to promoting physical health. _*This position offers competitive compensation and a $3,000 sign on bonus,*_

\*\*Responsibilities:\*\*

- Conduct comprehensive assessments to develop personalized treatment plans for patients.

- Administer therapeutic activities and exercises to enhance patients' functional abilities.

- Educate patients and their families on physical therapy's plan of care

- Collaborate with interdisciplinary teams

- Maintain accurate documentation of patient progress and treatment plans.

\*\*Qualifications:\*\*

- State licensure as a Physical Therapist.

- Excellent communication skills and ability to work effectively in a fast-paced environment.
