APP - Neurocritical Care/NeuroIR (Contingent) We are seeking a motivated Advanced Practice Provider (APP) with an interest in Neurocritical Care and Neuroendovascular services to join our Neurosciences team at Trinity Health Ann Arbor Hospital. Our hospital is a 500+ bed Level 1 trauma center, recognized by the Joint Commission as a Thrombectomy‑capable Stroke Center. We serve as a community teaching medical center for South Washtenaw County, Michigan, specifically the Ann Arbor and Ypsilanti areas. Full Time: Contingent Schedule: TBD Location: Ann Arbor, MI Specialty: Neurocritical Care and Neuro IR Requirements Master’s degree in Physician Assistant or Nursing (Requirement for NPs: AGACNP or ACNP only) State of Michigan Licensure – maintain active and in good standing throughout employment or eligible to obtain American Heart Association BLS – maintain active and in good standing throughout employment American Heart Association ACLS – maintain active and in good standing throughout employment Neurocritical care society ENLS certification and NIHSS before starting clinical work and maintained throughout employment Background Check – maintain active and in good standing throughout employment Successful completion of Credentialing by Medical Administration required Knowledge, Skills, and Abilities Available to work weekends and holidays with varying schedules. Able to interpret, adapt, and react calmly under stressful conditions. Able to recognize subtle changes in the patient’s condition and adapt care accordingly. Strong interpersonal, communication, collaboration, and conflict resolution skills. Can work independently and collaboratively with other disciplines using discretion and independent judgment. Can relate cooperatively and constructively with patients, families, and co‑workers. Can maintain the confidentiality of sensitive information. Ability to problem solve, formulate a plan of care, and evaluate the patient’s response to care. Can communicate within a multidisciplinary team approach to modify a neurological plan of care to attain expected outcomes. The Team Our team consists of four neurointensivists, three neurointerventional neurologists, and eight current APPs, along with a dedicated neurocritical care pharmacist and various levels of learners. Our APPs are vital to our team; they are skilled clinicians, proficient in procedural skills, and exceptional educators. They actively engage in education across multiple levels and disciplines and participate in research and quality improvement initiatives. The position includes a 2‑3 month onboarding process before the APP can work independently, regardless of prior critical care experience. The ideal candidate will provide clinical services for 12 shifts per month, with each shift lasting 13 hours. Shifts will be scheduled from 6 a.m. to 7 p.m. for day shifts and from 6 p.m. to 7 a.m. for night shifts. This position requires a mix of day and night shifts and includes holidays and weekends, which will be evenly distributed among the ICU APPs. Job Summary Primary responsibility in association with the Attending Physician(s) for the inpatient evaluation and management of critically ill adult neurologic patients (mainly stroke patients) in the intensive care setting/IR suite/ED. Participates in patients’ clinical care, demonstrating high levels of critical thinking skills as a member of an interdisciplinary team. Participates in the implementation of clinical practice guidelines (especially stroke related) for Advanced Practice Providers and serves as a resource for staff members, patients, and family. Responsibilities include completing history and physical examinations; ordering and interpreting radiological and laboratory test results; prescribing medications; documenting patient care services within EMR and departmental records; and performing procedures within scope of practice and demonstrated through competency (central lines, arterial lines). Job Specific Duties Conduct medical rounds (history, verification of medication, etc.), oversee and document the plan of care as part of the NeuroICU/NeuroIR team, and ensure best practices are in place. Perform focused neurological or comprehensive assessments and provide accurate documentation. Ensure proper communication between physicians and families. Serve as a patient advocate and educator and provide families with support and community resources. Provide patient information and effectively complete the hand‑off process to physicians and other APPs at shift change and patient reassignments 100% of the time. Maintain a high level of critical thinking and time‑management skills. The APP will serve on one of two different services, either neurocritical care or neurointerventional in daytime and cover both at night. Complete clinical documentation accurately and promptly within 24 hours. Document patients’ follow‑up per guidelines. Generate, collect, and integrate data from a wide variety of sources to make clinical judgments and critical decisions about the initiation of orders, procedures, referrals, and treatments. Write orders, monitor & alter drug therapies, and interpret diagnostic tests with education and experience within established protocol in collaborative relationship with a physician(s). Review all patient records before admission (labs, consents, advance directives, custody documentation, etc.) in a timely and accurate manner. Provide mentoring in precepting APPs, Medical Students, and Residents; actively give feedback and detailed descriptions of areas of improvement. Ensure zero‑based medication errors by efficiently and accurately ordering medication following the Five Patient Rights and Medication Administration Policies. Follow all Infection Control standards and ensure proper use of PPE per policy and procedures to ensure safe patient care. Provide education to patients and families on the continuing plan of care to include medications and side effects; update patients and families daily on the Plan of Care (POC). Evaluate and apply knowledge of cultural, socioeconomic, and age‑appropriate factors in assessments, treatment, and patient/family education. Accountable for measuring key metrics in the program (especially stroke program). Influence quality of patient care through research. Evaluate data, update database, and patient outcomes from research initiatives. Participate in gathering data for research and development, education, and implementation of system changes. Identify opportunities for improvement in patient gathering and evaluating quality data to implement QI projects and protocols. Collaborate with ICU Nursing Leadership to conduct an in‑service for RNs on particular area of expertise and promote awareness of best practices. Monitor and evaluate the appropriateness of care in the critically ill neurosurgical and neurological patients through collaboration within the multidisciplinary team by participating in daily ICU rounds. Communicate effectively within a multidisciplinary team to modify a neurological plan of care to attain expected outcomes. Cover clinical responsibilities while on call at the hospital day or night and communicate appropriately with the covering attending physician. Why Work with Us?
