Atrium Health Union West Transition Services and Internal Medicine is an outpatient transition services clinic that is committed to helping our patients restore their body and mind. Our Transition Clinic is a multidisciplinary ambulatory Internal Medicine clinic for patients at high risk of complications and readmissions due to medical complexity or barriers to healthcare access. We offer integrated visits with provider, pharmacist, and social worker. During our 30 day follow up period, we identify and alleviate barriers to health focusing on patient engagement, access, and education. Care is delivered in office, virtually and/or with community paramedicine support based on patient s needs. This is paired with a traditional Internal Medicine clinic. Get unrivaled clinical care, the highest level of support and a patient-first experience, in an environment designed for wellness.

As part of Atrium Health, we believe good health is a team effort. Our team is committed to providing the highest quality of medical care to our patients throughout all phases of their adult lives.

Highlights about the position include:

  • 100% Outpatient
  • Join current team of 1 Physician & 1 APP
  • Office hours Monday-Friday (Flex 7am-5pm)
  • Call rotation: Shared coverage rotated a week at a time; phone only
  • No Nights/Weekends

Ideal candidate:

  • Board- Eligible/ Board-Certified Internal Medicine Physician
  • Interest in Internal Medicine Seeing patients 18 years old and up
  • Passion for providing quality patient care in a highly productive clinic setting where collegiality and collaboration are a focus
