Summary Critical Time Intervention (CTI) is a care management service for adults with serious mental illness. The program serves up to 30 persons and coordinates services during a “critical time” of transition. Position A Licensed Practitioner of the Healing Arts (LPHA) with experience working with individuals who have mental health, or co‑occurring mental health and substance use disorders. Responsible for planning, direction, coordinating and overseeing all aspects of CTI services for the grant cycle. Provides supervision to staff, conducts team meetings, manages relationships with hospitals, runs regular case presentations, ensures continuity between fieldwork and meetings, reviews and manages caseloads, promotes model fidelity, staff productivity and staff wellness, monitors quality of documentation, updates resource lists, monitors phase dates, and assists in covering caseloads when needed. Essential Duties and Responsibilities Complete all CTI, agency and state required trainings. Ensure other CTI are compliant with trainings. Maintain, adhere to and adjust processes for communication, accessibility of the hospital for CTI staff, efficient and timely referrals, discharge and aftercare planning, access to client records as needed for CTI services and in-person engagement with individuals to begin CTI services as defined in the Memorandum of Understanding (MOU) developed with partnering hospital. Maintain, adhere to and adjust agreements developed with other systems of care assuring service continuity for individuals, such as the Local Department of Social Services, emergency service programs (crisis residences and crisis stabilization centers), housing agencies, local government units (LGU), correctional facilities, independent living centers, etc. Develop coordinated admission and transition plans with providers to assure service continuity with other systems of care in the community. Oversee CTI care coordination services and supports provided to promote community tenure and manage behavioral and physical health needs of individuals. Maintain and adjust the procedural manual in accordance with the most recent CTI Program Guidance issued by OMH. Maintain access to OMH’s CTI listserv to receive the latest requirements for CTI. Implement and train staff accordingly. Work closely with the Senior Director of Community Operations in the fiscal management and development of budgets for assigned services to review profit and loss analysis and work with staff to ensure spending is monitored and adhered to. Approve service dollar requests adhering to program guidelines. Maintain regular staff contact to promote the philosophies of the agency. Facilitate weekly team meetings with clearly defined procedures to ensure that all necessary program information, training and agency policies and procedures are reviewed. The Team Leader facilitates these formal group supervisions to review referrals and status, share resources or expertise in specific areas, review case presentations and caseloads discussing the phases and needs of individuals, review safety concerns, etc. Monitor and review all documentation in the electronic health record to ensure compliance with the CTI manual and agency policies and procedures. Oversee the development of quality individual service plans and the establishment, review and maintenance of client care protocols ensuring that all state policies, guidelines and regulations are met. Ensure client confidentiality is maintained at all times. Responsible for the resolution of personnel issues. Complete staff evaluations in accordance with agency policies. Maintain time cards and attendance of staff. Interview, hire, reward and discipline staff in conjunction with the Senior Director of Community Operations. Participate in the development of staff and identify their training needs as required. In conjunction with the Senior Director of Community Operations, review annual satisfaction surveys and progress towards established goals and objectives, and participate in developing a plan for ongoing improvement of services. Collect data and perform compliance and quality assurance activities as required by the grant and agency. Complete reports/input data in reporting systems as directed by the Senior Director of Community Operations. Develop and implement corrective action plans as needed. Ensure staff have access to the Center for Practice Innovations (CPI) Care Management Institute, I-CONECT for required CTI training. Track additional best-practice trainings for staff that include trauma-informed care, individual engagement, cultural sensitivity, effective communication, motivational interviewing and harm reduction principles. Participate in OMH-sponsored CTI Learning Communities/Collaboratives. Attend state and regional level meetings to review progress, outcomes, and develop best practices for CTI in NYS. Review caseloads, facilitate case conferences to problem solve treatment challenges and track CTI phases of clients to adhere to CTI model fidelity utilizing the Phase-Date Form. Develop safety protocols for staff and clients. Oversee all medically necessary CTI services to include Person-Centered Planning, Psychosocial Rehabilitation Services, Crisis Prevention Services, Health Services and Care Coordination Services. Review all discharge plans for transition from the CTI program to other supports and services or to self-care. Functions as a member of the multi-disciplinary team. Other related duties as assigned. Supervisory Responsibilities Directly supervise 2-3 CTI care managers. Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include but are not limited to interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. Education and Experience Required to have a current valid license as a: Physician: An individual currently licensed to practice medicine issued by NYSED Physician Assistant: An individual currently registered to practice as a physician assistant issued by NYSED Nurse Practitioner of Psychiatry: An individual currently certified to practice as a nurse practitioner issued by NYSED Registered Nurse (RN): An individual currently licensed to practice as a registered professional nurse issued by NYSED Psychologist: An individual currently licensed to practice as a psychologist issued by NYSED Social Worker: An individual who is either currently a licensed clinical social worker (LCSW) issued by NYSED Licensed Master of Social Work (LMSW) practicing under the supervision of a psychiatrist, psychologist, or LCSW employed by the agency Mental Health Counselor: An individual currently licensed to practice a mental health counselor issued by NYSED Marriage and Family Therapist: An individual currently licensed to practice as a marriage and family therapist issued by NYSED Psychoanalyst: An individual currently licensed to practice as a psychoanalyst issued by NYSED Creative Arts Therapist: An individual currently licensed to practice as a creative arts therapist issued by NYSED Preferred: 1-2 years supervisory experience. #J-18808-Ljbffr
